Abstract

The Unified Health System (SUS) is the largest social policy guaranteeing access to healthcare
in Brazil, with the aim of promoting universality, equity, and comprehensiveness in the services
it provides. However, this system faces obstacles that hinder the achievement of its objectives.
This paper aims to explain the evolution and main challenges faced by the SUS throughout its
existence. This is a narrative review that aims to highlight the challenges faced by the Unified
Health System. The searches were conducted in the databases Scielo, Google Scholar, and
Lillacs databases, using the keywords “Unified Health System,” “obstacles,” “challenges,” and
“reality,” including works, original studies, and reviews that addressed these challenges. The
main difficulties faced include chronic underfunding, regional inequalities, obstacles in
management and administrative efficiency, and the devaluation of healthcare professionals.
